DEA Number Required for TARs

Effective December 15, 2008, Pharmacy providers must include the prescriber’s Drug Enforcement Agency (DEA) number for all Schedule II and Schedule III Controlled Substance Drugs on Treatment Authorization Requests (TARs).

The DEA number should be entered in the Prescriber ID (License number ) field for electronic TAR (eTAR) and National Council for Prescription Drug Programs (NCPDP) submissions. On paper TARs, the number should be placed in the Medical Justification area.

TARs for Schedule II and III drugs submitted without the DEA number will be deferred for the DEA number.
