Beginning April 23, 2007 through June 24, 2007, Medi-Cal will accept both versions of the professional claim form (the HCFA 1500 or the CMS-1500) and the institutional claim form (the UB-92 or the UB-04). During this period, providers are encouraged to transition to the new form and deplete their HCFA-1500 and UB-92 form stock, in preparation for exclusive use of the CMS-1500 and UB-04. Beginning June 25, 2007, Medi-Cal will only accept the CMS-1500 and UB-04 forms.
