The PDF version of the NPI Application/Update Form (CMS-10114) is no longer available on the Centers for Medicare & Medicaid Services (CMS) and National Plan and Provider Enumeration System (NPPES) Web sites. Covered health care providers can still obtain an NPI through one of following three options:
1) Web application, 2) electronic file interchange (EFI) and 3) paper application.

1. For the fastest and most efficient process, complete the online application available on the NPPES Web site.
2. Authorize a third-party Electronic File Interchange Organization (EFIO) to submit the application information via the bulk enumeration process.
3. Contact the NPI Enumerator to request a paper application. Providers should return the completed, signed application to the NPI Enumerator at the address below.
