TAR Policy Update for Split-Billed MRI or MRA Services

Effective for dates of services on or after August 1, 2008, new guidelines for Treatment Authorization Request (TAR) and electronic TAR (eTAR) submissions apply for split-billed Magnetic Resonance Imaging (MRI) or Magnetic Resonance Angiography (MRA) services. Providers must submit TARs in the following manner:
- If a submitting provider and rendering provider submit claims on MRA or MRIs services, the TAR should be submitted with two service lines. The first line should display the CPT-4 code and the modifier TC (technical component). The second line should display the CPT-4 code and the modifier 26 (professional component). This allows both providers to bill with the same TAR.
- If one provider rendered both professional and technical services, the TAR should be completed using one line with the modifier ZS.
