The Adjudication Response (AR) notice displays Treatment Authorization Request (TAR) service information necessary for submitting claims or responding to requests from the Medi-Cal field office. ARs are mailed to a provider’s address on file with the Department of Health Care Services (DHCS). To request additional copies, providers should contact the Telephone Service Center (TSC) at 1-800-541-5555 and then follow the TAR prompts. AR copies are mailed to a provider’s address on file or sent by fax to the number submitted on the TAR.
