Upon NPI implementation, select providers will have multiple addresses linked to their National Provider Identifier (NPI). Providers should continue to log into the “Transaction Services” menu. If the NPI number has multiple addresses, the “Provider Address Selection Options” page will appear. After selecting the appropriate address from this page, the provider can continue with the eTAR submission.

If the provider does not have multiple addresses linked to the NPI number, the “Provider Address Selection Options” page will not appear and the provider will continue with the eTAR submission.
