System Issue Regarding eTAR Attachments

Electronic Treatment Authorization Requests (eTARs) submitted from March 24, 2017, through March 27, 2017, and from April 14, 2017, through April 17, 2017, experienced a system issue involving uploaded and faxed attachments. If an eTAR attachment was submitted within these dates and the status of the eTAR indicates “In Review” or “Deferred,” please upload or fax the attachment again.

If you experience issues with uploading or faxing eTAR attachments or have any questions regarding this issue, please call the Telephone Service Center at 1-800-541-5555, option 4, followed by option 2.
