Treatment Authorization Requests (TARs) that were authorized using the old Z8500 code with a through authorization date on or after December 1, 2008  will not be denied in claims processing. There are Special Processing Guidelines (SPG) to resolve this issue. The provider must bill with the code that is authorized on the TAR.

TARs that request authorization using the new S5102 code prior to the December 1, 2008 effective date will not be processed.  The provider can take one of the following two actions:

• Submit a TAR requesting authorization of Z8500 for service periods prior to December 1, 2008 and another TAR requesting authorization of S5102 for service periods on or after December 1, 2008; or
• Submit a TAR requesting authorization of Z8500 that covers a service period before and after December 1, 2008 (e.g., service period of November 1, 2008 through April 30, 2009).
