Reminder: CHDP Code Conversion and Claim Form Transition Implementation

The Child Health and Disability Prevention (CHDP) code conversion and claim form transition is on schedule to implement. This transition will impact all CHDP services billed with a date of service on or after July 1, 2017. To prevent unnecessary delays or denials in claim processing, providers may review the following previously published NewsFlash articles that outline changes and information for billing CHDP services after the transition date:

Note:  At this time there will be no changes for school-based providers billing CHDP services. School-based providers will continue to bill laboratory, vaccine and health assessment services using the Confidential Screening/Billing Report (PM 160) claim form until further notice.

Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs) and Indian Health Services – Memorandum of Agreement (IHS-MOA) 638, Clinics will continue to bill using the UB-04 claim with the Confidential Screening/Billing Report Information Only (PM 160 Information Only) for reporting purposes, as required.
