PCPs complete online forms in order to:

• Certify recipient eligibility.
• Obtain a recipient ID number for billing.
• Enter screening results of all recipients.
• Enter diagnostic procedures, work-up status, final diagnosis and treatment information, as required, of recipients with abnormal screening results.

PCPs must enter data within 10 days of receiving final results.
