Accessing Online Forms
Purpose
PCPs complete online forms in order to:

  • Certify recipient eligibility
  • Obtain a recipient ID number for billing
  • Enter screening results of all recipients
  • Enter diagnostic procedures, diagnosis status, final diagnosis and, if applicable, staging and treatment information of recipients with abnormal screening results
